My wife and I want to order the reports for the donor we have chosen. As I was reading the information for the shipment of the sperm I noticed that there needs to be paperwork signed by the doctor and signed by us. I don't have a printer how can I get them sent to me or request them be sent to us. Thank you so much for your time. Have a great day.

    We can fax the A-1 form directly to your physician for you if you enter the information on your My Account page.

    You can place your order prior to us receiving the signed A-1, but we need it on file to schedule your shipment.

    If you call into our Client Services Department at 866-927-9622 and we can fax or mail you the paperwork you need to sign to open your account.
