A1 Form

Hi Scott,

I am in the process of getting routine blood tests and preparing for pregnancy, and I have already ordered my vials. However, I will not have my A1 form submitted until I actually see the doctor sometime next month or so. There are no concerns with that, are there? It is my understanding that you do not need the form submitted until prior to shipment. Just want to make sure. Thanks!

    Hi Gltrbug -

    Yes, we need the A1 signed prior to shipment. If it is easier you can simply fax it over to the doctor via our website.
