Hello

I'm a little bit confused on what's the difference between subscribing to the different levels of information vs. downloading the separate pieces of information below the subscription options.

The list of information below each subscription level looks like an ala carte menu, vs choosing the subscription option. Is this the case, where you can just choose to down load that information on a case by case basis?

If you purchase the subscription is this something you pay on an annual basis and then have access to all information on all donors.

It's kind of confusing....please provide some more guidance!

    The Donor Information Subscriptions give you 3 months of access to all information avaiable on all donors. This access is view only. The list of items tell you what is included in each level. Please note that Level III includes all the items in Level II in addition to the Donor Conversations, Keirsey Reports, etc.

    If you choose to purchase a la carte items for an individual donor(s), they are downloaded and you have permanant access.

    Remember, if you purchase a Subscription, we also send you a Keepsake Packet (hard copies) of your donor's info once you report your pregnancy (equal to your subscription level).

    So I can purchase the ala caret options without doing the subscription?

    Also, when sending in my AI form does anything other than my name and basically what is on the form need to be included with that? I have set up an account on line but need to have the physician form signed and return.

    Yes, you can purchase individual items without buying a Subscription.

    You will also need to fill out the A2 and A3 forms to place your order. These forms establish your purchase and storage acount with CCB.

    The online account only allows you to purchase donor info, save searches, save favorite donors, etc. Purchasing and shipping vials requires the A2-A3.

    Thank for your reply. For the A1 form, when my physician sends in that information via fax what additional information, if any needs to be included with that form, other than just my name. I understand the other forms need to be completed as well prior to ordering but I am specifically inquiring about the A1 physician consent form.

    I purchased a Level II subscription a few months ago, purchased and stored a few vials and the subscription has since expired. I'm wondering if I do in fact get pregnant, will I receive the hard copy of my donor's information? Will I need to purchase my donor's information ala carte? How exactly does that work? It is definitely information I'd like to have.

    Yes. Because you took advantage of the D
    Onor Subscription, once you report your pregnancy we will send you a Keepsake packet with hard copies of your donor's Level II info.
