Shipping vial to another person

I'm planning on selling my one remaining vial (unfortunately, it left CCB when I ordered two vials for a back to back IUI and was only able to use one so I don't believe it qualifies for the buy back program). Is there anything I need to do in order to have it shipped to their facility of choice? I know for a fact I won't be using the vial in the future.

    For legal reasons, we will need to transfer the vial into the other client's account in order to then ship it to their physician. There is a form you will both need to sign. Please contact Carmen in our Client Services Department at 866-927-9622. She will be glad to help you.
