Question regarding vial transfers

I am in the process of trying to transfer two vials from my storage to another CCB client's storage. She mailed me a transfer form that she signed and told me that I needed to download and sign one as well and then mail them both to CCB. I have not been able to find the form on your site however. Could you please direct me to the necessary form? Thank you in advance!

    The transfer form is not on the website.

    You have two options: You can either use the same form below the other person's info (but you will need to get it notorized again); or, you can contact our Client Services Department at 866-927-9622 and they can email or fax a blank form to you.
