authorization form

Hello everyone :)
For anybody that is doing an at home insemination did anyone else run into problems having a doctor sign the authorization form.
Maybe I'm explaining it wrong when I'm speaking to the doctors I have talked to. Most doctors have said they feel uncomfortable signing it.
Any ideas or hints on how I should word or explain myself and the form better.

I would suggest finding another doctor. CC has a great reputation, and my doctor signed the forms with no issue at all. The forms are self-explanatory, so there should be no need for an explanation. Also, a doctor who is experienced should have no issue with signing them…unless they have some sort of contract with another clinic.
