Getting Started

I am just getting started and I am unsure what to do first. I have a doctor, but do I have to get them to "sign off" on this? I understand needing to be under a doctor's care, but I registered with another sperm bank and they have an option for home insemination. Do you have that?

    Congrats on taking the first step!

    We do require your medical professional to sign an authorization form before you can ship out any specimen. You can ship vials to your home, as long as your physician fills out the A1 form accordingly. Some of our clients coordinate home inseminations with a licensed midwife, who can also sign this form for you.

    As you search for donors, look for donors who have ICI vials available. These are the vials that you would need for home insemination.

    Most clients start the process by searching for donors and downloading the free donor information. Our donor subscriptions offer additional detailed information on all of our donors which may help you narrow down your options.

    Before you can purchase vials, you will need to complete the A2 and A3 forms, in addition to having your medical professional send us the A1 form mentioned above. You can find these forms here: http://www.cryobank.com/How-It-Works/Opening-Your-Account/
