physician signature

I am insured through Kaiser and am trying to get the required form signed. Just last month I went to my primary physcian for my annual physical and got a pap smear. At the visit I informed her of my plans for purchasing donor sperm and needing the form signed. She said I would need to see ob/gyn offices for that. I went for my appt at ob/gyn today and that doctor said she was not authorized to sign it and to speak with my primary since she is the doctor that has been caring for me. What physician do I need the signature from?

    It should be signed by the physician that will be handling your insemination. (It doesn't matter to us if it's your primary or OB/GYN.) It is also important to include the correct shipping address on the A1 form.

  • I am not sure of your exact situation but I also have Kaiser and was referred to the Reproductive Endrocrinology and Infertility Department by my ob/gyn. We met with one of the doctors there and he signed the form. Hope this helps!
  • I plan on doing an at home insemination so I do not have a doctor performing the procedure.

    The A1 form can also be signed by another type of medical professional, like a nurse practitioner or licensed midwife. Please note that we don't provide instructions or equipment for home insemination. Ideally, the medical professional that signs your form should be the person that you would consult with regarding your home insemination.
