physician signature!!!!

Has anyone else had an issue getting their doctor to sign the release form. I am insured through Kaiser and am trying to get the required form signed. Just last month I went to my primary physcian for my annual physical and got a pap smear. At the visit I informed her of my plans for purchasing donor sperm and needing the form signed. She said I would need to see ob/gyn offices for that. I went for my appt at ob/gyn today and that doctor said she was not authorized to sign it and to speak with my primary since she is the doctor that has been caring for me.

I personally did not have this problem, but I was afraid it could've happened so I looked for other sites that didn't require a signature from your Dr. .....Try nwcryobank.com.... prices were cheaper, but they have limited donors...
