physician signature!!!!

Has anyone else had an issue getting their doctor to sign the release form. I am insured through Kaiser and am trying to get the required form signed. Just last month I went to my primary physcian for my annual physical and got a pap smear. At the visit I informed her of my plans for purchasing donor sperm and needing the form signed. She said I would need to see ob/gyn offices for that. I went for my appt at ob/gyn today and that doctor said she was not authorized to sign it and to speak with my primary since she is the doctor that has been caring for me.

    Nope, my gyno doesn't perform it, so i went through the Houston IVF clinic and they saw me in Feb 2015 and ran some test then then this mth i had my first IUI. So no i never had any issues.
