If I purchased ICI premium and the thaw count was around 10 million but not over 10 million...
Is it my responsibility to let CCB know
Or should the Doctors office have been the one
to contact CCB ?

You will need to contact us yourself, although your doctor will have to fill out the paperwork, if you want to file a substandard specimen report. Please note that counts can vary up to 20% based on the lab and technique used. Contact mevans@cryobank.com for more help.
