Physician Authorization Form

Hi, Brand new here, and signing up, I see that there is the physician form.
If I am planning on only using ICI, and doing AI at home, am I required to have this form filled out?

    Yes, you would need a medical professional to sign the Physician Authorization Form (A1) before you can ship vials to your home. You might want to ask your OB/GYN or a licensed midwife.

    Please keep in mind that we only provide the specimen, so you would need to consult with your medical professional if you have any questions about what supplies you will need, cycle timing, etc.
