We also include the donor's Summary of Records which includes infectious disease testing results and shows that he meets FDA donor eligibility requirements. We also send the tank handling & thawing instructions.
The Donor Profile, Medical History, and Genetic Test Summary (GTS) are free info items that you can download at anytime from your donor's profile page. https://cryobank.com/donor-search/donor-information/
You are welcome to provide that info to your doctor. If your physician has recommended expanded carrier screening for you, he/she will probably ask you for your donor's GTS.
