is local pick-up possible?

Hello,

I live in New York City and would like to know if it is possible to personally pick-up purchased vials. I was unable to find information reg. this option and on costs.

Also, I am planning on IUI in June. What do I need to provide in order to have it all ready to go? Please advise.

Could you please provide me with any helpful information on procedure.

    Yes, if you decide to purchase and store vials, you do have the option to have them transferred to our New York branch. We transfer vials to our branches twice a week. This option usually works best with our Family Today/Family Tomorrow programs.

    There are some forms that you and your physician would need to complete beforehand. (Your physician will need to sign a form allowing you to pick up vials from our facility.)

    I would suggest to start this process at least two weeks before you intend to use the vials.

    Please feel free to contact Client Services at 866-927-9622. They will be able to give you more details about the logistics, and provide you with the price which is based on the vial type, vial quantity, and ship date.

    CCB never seems to know what's going on when I call. Because I always get information, I tend to call twice to ask different people the same questions. I called a few weeks ago because I will likely need to pick 2 vials up from the NYC facility next week. My vials are already there. I have never had my dr sign forms saying I would be picking them up.

    Both times I called CCB they only said I needed to call 24 hours before I planned on picked up my vials. Are you now telling me this is incorrect? Because I will be really angry if I can't get my vials next week.

    I suggested to Rumi to allow two weeks for the entire process. Once the transfer of the vials to your local branch is complete, you can call 24 hours (1 business day) before to set up the pick-up.

    The pick-up times at the NY branch are Monday-Friday, 9am to 5pm. (If you need a morning pick-up for the following business day, just please make sure that you contact us before 1pm EST.)

    The authorization form (A1), is the form that your physician signs before you can ship out any vials. He/she indicates if you are allowed to do pick-ups. Please feel free to contact me directly if you have a question about the authorization form that we have on file for you.
