Same sex couple

We are a same sex female couple, we both want to use the same sperm, do we both turn in the physical form in order to do this?

    If you plan to take turns going through the insemination process, the person that will be inseminated first should set up the account under their name. The A2 &A3 forms can be completed online and your physician will need to complete the A1 form.

    When the second person is ready to start the insemination process, they will have to set up an account under their name (complete A1, A2, & A3 forms)and we can do a free Same Family Transfer, where we transfer the remaining vials and remaining storage time to the second account.

    If you plan to go through the insemination process at the same time, you will each need your own account. You would each need to complete the A2 & A3 and your physician would need to complete the A1 form. You would purchase vials for each account.
