URGENT! 2nd bout with testicular cancer. considering a new sample, is it worth it?

10 years ago, I was diagnosed with testicular cancer and advised to go to a sperm bank, Now i am considering leaving a new sample, the vials from my prior sample still exist, i was told that the samples had a low count, But i am wondering exactly how low it is. I know nothing about this topic as me and my girlfriend are now just thinking about this being that i have to have my other testicle removed due to cancer, The doctor talked to me about sperm banking again, but i have had 2 surgeries on my remaining testicle and chemo 10 years ago, Single dose carboplatin. tomorrow is my surgery, so i must do this today or tomorrow to morning if i am going to.

Here is the data for the vials I have stored.

Specimen date : 12/04/06
Post thaw motility %:20
Total Motile cells/dose (million) 0.45
Prep type : ICI
Dose balance : 6

My biggest question is what are the chances successful insemination with these vials, And with my history of 2 invasive surgeries on the testicle itself after my chemotherapy, a biopsy of testicular tissue on the remaining testicle. as well as an orchidopexy. of that testicle.
