International Authorization (IAS) or Physician Authorization (A1 Form)
I am from South Korea and living in Seoul, South Korea.
My nationality is also South Korea.
I have got a mail from CCB that I have to get International Authorization (IAS) or Physician Authorization (A1 Form) to buy a vial.
Could you ship vials to South Korea if I get IAS?
I am a licensed physician in South Korea training subspeciality as a fellow in a hospital.
Can I fill up my own IAS?
Or should I have to ask other physician in South Korea?

Welcome @LaminaG ! We have recently shipped vials to the Maria Fertility Hospital in Seoul.
For international shipments, we can only ship to medical facility addresses and not home addresses. (Please keep in mind that our shipping tanks are validated to keep the specimen frozen for 7 days. Most fertility clinics have cryogenic storage to properly store the vials for a longer period of time.)
You would need to complete the IAS online prior to scheduling a shipment. It will ask you for your physician's information and you can enter your own information. After you submit your IAS online, we will review it.When we encounter a physician that hasn't worked with us before, we will follow up to get additional information. If for some reason we would not be able to add you as an approved physician, we would let you know.
