Vial transfer

I would like to gift the remaining vial I purchased from CCB to a friend and her wife. The vial is being stored at my fertility clinic and not at CCB, so the non-family transfer of ownership form doesn’t exactly fit the situation. I want to know if A) this is possible and B) what form(s) are required to complete the transfer to their fertility clinic. The donor is no longer active, if that makes a difference.

    @sbs1999 We cannot assist with the transfer of donor vials that left our facility. It's important to contact us to check if the donor is limited for sibling use and also if there is any new medical/genetic information for that donor that would be relevant to someone planning a pregnancy.

    Some clinics will not accept vials that are not shipped directly from a sperm bank, so I would also suggest having your friend check with her clinic to see if they would accept this type of shipment. If so, they may have recommendations on how to transport the vial.

    Also, if your friend becomes pregnant, it's important for her to report her pregnancy so we have this information in our system: https://cryobank.com/services/post-conception-services/report-pregnancy/
