ICI for home use

I've just ordered vials of ICI with the intent to try at home insemination first. Do I still need the physician authorization form? My doctor said she's had many patients do ICI at home and they never needed a form before. Also, do I have to do anything different to arrange shipping to my home?

    Yes, we require an A1 form signed by your medical professional before you can schedule a shipment. Please note that we only provide the specimen and we do not provide any medical supplies or information on how to do an ICI at home. (We recommend checking with your medical professional for that info.)

    A signature is required for shipments, so it's important to make sure that someone will be home to sign for the shipment. We can also ship to a FedEx location, but you will need to contact Client Services to arrange that.
