Vial Exchange Program

Hello,

I had a couple of questions regarding the Vial Exchange Program:

1. Do you need to call in, or do you just send in the CCB Vial Exchange Form?

2. Also, does someone contact you regarding payment, or is the administrative fee of $150 automatically charged to your credit card on file?

    Hi Tara-

    You can call in or send in the form and we will contact you.

    The fee can be charged to your card on file or you can give us a different card. If you do not want it charged to your current card, please let us know.
