If I had my A1 form completed a few months ago do I need to have it resubmitted if I want to have it shipped to another address or can I just call and tell them the new shipping address?

    Hi Jen -

    We can only ship to addresses that your physician approved on the A1 form, so if you have a different shipping address, you will need to contact your physician and have her/him submit a new A1 form to us.

    i had called and asked this question to the staff and they told me that if i had my address on the A1 form and not my doctors office then i could change the address when i called for a scheduled delivery. Is that incorrect because i was going to have a vial shipped in less than two weeks from now. Also, is it possible to have two addresses on file for you so that if i will not be at home to sign for the package i can have it sent to the default address?

    Hi O-

    If your physician authorized shipments to your home on the A1 form, you can ship to both your physician's address and your home address. If your home address changes, you can update that with us.

    If you won't be home to sign for the package, you have the option to ship it to a FedEx location for pick-up at your convenience. Just confirm beforehand with your local FedEx, that they have a hold for pick-up option and find out what their pick-up hours are. When you call to schedule your shipment, you can provide us with the FedEx office address.
