A3 FORM

Hi CCB,

How can i send the A3 form back to you? should i just fax it and thats it? Is there anything do i need before i can purchase?

    Hi Hmp -

    You can fax the A3 form to 866-625-7336. Please include the A2 form with it. Once we receive these forms, it will take us one business day to process them and then you will be able to place your vial order.

    Before you are able to ship vials, we will also need an A1 form from your medical professional. If need a copy of this form, please go to
    MY ACCOUNT --> FORMS
