I was wondering on the newer form ( typed not hand written ) where would i find if the donor has freckles? And how many pregnancies or births has # 12647 had? Thank you

    Hi Tracy -

    If the donor chooses, he may add freckles at the bottom of page 5 under distinguishing features "other".

    If a donor has distinguishing freckles, it will also be listed on page one of his Facial Features Report.

    12647 has less than 5 pregnancies and no births reported yet, but has only been available for about two months.
